What FreshMedLeads is

FreshMedLeads sends email digests and alerts summarizing newly registered medical practices and facilities published in the CMS NPPES registry, a public U.S. government database. We are not affiliated with CMS or HHS.

The free digest

The free weekly digest costs nothing, requires no payment information, and you can unsubscribe at any time using the link in any email.

About the data

Where it comes from. Every field we deliver is taken directly from the CMS NPPES registry, a public U.S. government database. Every CMS field passes through unchanged — we do not edit it, correct it, or overwrite it. Nothing is bought or appended from any outside source: no other vendor, no data broker, no scraped web data, no email addresses. That promise is absolute and it is the one to hold us to.

The one column that is ours. Alongside the twelve CMS fields, your file carries a thirteenth column, Data Note, holding our own quality flags. They are computed from that same federal file and nothing else, and they only ever add a caveat — they never remove a row, never alter a CMS value, and never introduce information CMS did not publish. There are four: "phone unverified" when the number is not in a dialable US format; "shared phone (N businesses)" when one number answers for five or more different business names; "duplicate listing" when the same organization, street, city and specialty appear on more than one row; and "N locations in this file - established operator" when one organization holds three or more registrations in the file, so you know it is a chain rather than a new independent practice. About nine rows in ten carry no flag at all — measured, and it moves a little as the file does. Ignore the column and you still have exactly what CMS published. We are not affiliated with CMS or HHS.

How current it is. CMS republishes the registry on a weekly cycle, normally Monday. That means the newest records in any file can already be several days old, and a practice that registered yesterday will not appear until the next federal file lands. Where we say a list covers registrations "in the last 30 days," that window is measured to the date your file is generated, against the most recent federal file we hold.

Record counts are estimates until your file is built. Any number quoted in an email, advertisement, sample, or product page is a count taken at the moment it was calculated. It will not match your delivered file exactly. The 30-day window moves every day, CMS republishes weekly, and records are added and withdrawn between those two points. The file you receive is the authoritative count. If your file arrives more than 15% smaller than the figure we quoted you, tell us and we will make it right — by widening the window, replacing the file, or refunding it, whichever you prefer.

One organization can appear more than once. CMS issues an NPI per enrollment, not per company, so a single organization can hold many separate registration records — typically multi-site groups, hospital districts, retail pharmacy chains, and practices that register several locations or specialties at once. In a recent file, roughly one row in ten shared an organization name with another row, and a single hospital district accounted for 27 rows on its own. You are buying registration records, not a count of unique businesses, and we do not de-duplicate them, because collapsing them would hide genuinely separate locations with separate decision-makers. If you would prefer a de-duplicated file, ask and we will build one.

What is not included. The federal registry does not publish email addresses, so no list from us contains one. Phone numbers, addresses, specialties, and decision-maker names are as the provider self-reported them to CMS, and some will be out of date, mis-keyed, or belong to a billing office rather than the practice itself.

Corrections and re-sends. If we find an error in something already delivered, we may send you a corrected file. Where you receive more than one file for a single order, the most recent one supersedes the earlier one and is the file to use. We will always say why a second file was sent.

Provided "as is." Government records contain errors and reflect what providers self-report when they register. We make no warranty about the accuracy, completeness, or timeliness of the data, and you should verify any record before relying on it for a business decision. You are responsible for complying with applicable laws — including telemarketing, fax, and anti-spam laws — in how you use any contact information we supply. To the extent the law allows, we disclaim the implied warranties of merchantability and fitness for a particular purpose. We are not liable for indirect, incidental, special or consequential loss, or for lost profits, lost business or wasted staff time, even if we were told they were possible. Our total liability to you for any claim arising out of the service is limited to the amount you paid us in the twelve months before the claim.

What we sell

Two lists, and two ways to buy either one.

Every specialty is included at every price. There is nothing to filter, and nothing held back for a higher tier.

The one-time file is a catch-up. The subscription looks forward. A one-time purchase is a single file of every practice registered in the last 30 days, emailed to you immediately — a snapshot, not an archive and not a feed. It never repeats and it never renews. A subscription sends a fresh sheet every Monday holding the records that are new since your last sheet, in the same states, with the same every-specialty scope. It does not include the back catalogue.

You can hold both, and they are built to sit together. Buy the one-time file for the last 30 days, then let a subscription carry it forward from that point. Buying one does not include the other, and cancelling a subscription changes nothing about a one-time file you already bought.

Delivery of a one-time file is automatic and normally takes a few minutes. Your file is built and emailed the moment your order comes through.

One exception, and it is deliberate. If your state holds fewer than ten records in the window you bought, we do not send the file automatically. You get an email from a person instead, telling you the real number and offering you the choice: take it anyway, widen the window, switch to another state, or have your money back in full. Nobody should pay $99 and open a file with four rows in it. This is the only case where a one-time order does not deliver by itself.

If you subscribe

What arrives, and when. Every Monday we build your sheet and email it as a CSV: the records added since your last sheet, for your state, or for all 50 if you are on National. Every specialty, every week. Same columns as the one-time file — practice name, specialty, phone, address, decision-maker name and title, and the Data Note column described above.

A quiet week means no sheet, not an empty one. Our sender refuses to mail a file with nothing in it. If the registry added nothing in your states that week, you get a short note saying so instead of a CSV. Nothing is lost — the next sheet reaches back to where your last one ended and picks those records up, and we cap that catch-up at 30 days so nobody is ever handed one enormous file. Be clear about what that means for billing: you are buying a month of coverage, not a sheet. A quiet week is still a billed month, and we do not pro-rate it or credit it. If your state turns out to be quieter than you want, tell us and we will move you to National for the difference, or you can cancel.

Billing. A subscription is charged monthly, in advance, to the card you used at checkout, and it renews automatically until you cancel. Payment is handled by Stripe; we never see your card number. The price you sign up at is the price you keep, unless we email you about a change first.

How to cancel. Cancel yourself, any time, in the billing portal — enter the email address you subscribed with and Stripe sends you a sign-in link. It takes effect immediately for future charges, and you do not have to wait for us. You can also email leads@freshmedleads.com from the address you subscribed with and a person will do it for you and confirm in writing; if you cancel that way, allow one working day. Cancelling ends future charges and future sheets. There is no term, no notice period and no cancellation fee.

Cancelling does not refund the period you are in. You keep receiving Monday sheets to the end of the month you have already paid for, and then it stops. We do not refund months already run.

Sheets you already have stay yours. Cancelling takes nothing back. Every file we delivered remains yours to use under the license below, permanently, with no expiry and nothing to switch off. Keep your own copies, though: if you lose one we will re-send it when we still hold it, but that is a courtesy, not a guarantee. We are not your archive.

Tax

Prices are exclusive of any applicable sales, use or similar tax. Where we are required to collect it, it will be added at checkout and shown before you pay. We do not currently add tax to any order.

Refunds

One-time files. Each file is generated to order and delivered digitally, so orders can't be refunded once the file has been delivered — with one exception: if your file arrives more than 15% smaller than the count quoted on freshmedleads.com, you choose a wider window, a replacement, or a refund. If the file is defective or isn't what you ordered, we fix or replace it free. Before delivery: full refund, no questions — just reply to your receipt.

Subscriptions. Cancel any time; there is no long-term commitment and no minimum number of months. Cancelling stops the next charge. It does not refund the period you are currently in, and we do not refund earlier ones. If a weekly sheet is defective, mis-built, or plainly not what your subscription covers, we fix or replace it free.

Either way, one address: leads@freshmedleads.com. A human reads it.

What you may do with it — your license

You are buying a license to use records, not ownership of the data. Every file or sheet we deliver comes with a license that is non-exclusive, non-transferable and permanent: use it for your own business, for as long as you like, including after a subscription ends. Nothing expires and nothing is deactivated.

What the license does not cover: reselling, republishing, sublicensing or systematically redistributing our lists, sheets, alerts or data feeds, or building them into a product or feed you supply to someone else. That needs written permission from us. Using a file across your own company is fine; handing it to another company is not.

This license covers the files we compile. It does not cover the underlying federal registry, which anyone — including you — can download from CMS for free.

Not a consumer report

What we sell is business registration data from a public federal registry. It is not a consumer report, and we are not a consumer reporting agency under the Fair Credit Reporting Act. You may not use any part of it for an FCRA-permissible purpose: no credit decisions, no insurance underwriting, no employment or volunteer screening, and no tenant or housing screening. If that is what you need, buy from an FCRA-compliant provider instead. We will not knowingly supply a buyer who tells us that is the plan.

How you use it is on you

You are responsible for following the law every time you use a number or an address we supplied — telemarketing and TCPA rules, do-not-call lists, fax rules, and anti-spam law. We supply records only. We do not call, text, fax or email anyone on your behalf, and we do not offer that service at any price.

Which leads to the part most list sellers leave out, so here it is plainly: if your use of the data lands us in trouble, you cover us. You agree to indemnify, defend and hold harmless LEADTHEM LLC, its members and its staff against any claim, demand, investigation, penalty, fine, judgment, loss or expense — including reasonable legal fees and the cost of defending it — arising out of how you used a list, sheet or record we supplied, or out of your breach of these terms. TCPA and do-not-call claims are the obvious case, and an ongoing subscription is exactly where they build up. We will tell you promptly if something like that arrives and will cooperate with you; you may take over the defence with counsel we reasonably approve, and you may not settle in a way that costs us money or admits anything on our behalf without our written agreement. This clause survives the end of your subscription and of these terms.

Changes, and which document wins

If these terms change materially, we'll note it in the email digest. Continued use after a change means you accept the updated terms.

These terms govern. Where anything on a sales platform conflicts with this page — freshmedleads.com itself, a Stripe checkout screen, a marketplace listing, an advertisement, a sample, or an email from us — this page controls and supersedes it. Platform rules still govern the payment itself; they do not change what you bought. Nothing here removes a right you hold under law that cannot be waived by agreement.

These terms are governed by the laws of the State of Michigan, USA.
