20 records, nothing behind a padlock.

Everywhere else on this site the phone number and the decision-maker are behind a padlock, which is fair — that is the part you are paying for — but it means you have never actually seen a finished record. Here are 20 of them, straight out of the same pipeline that builds every paid file, with every paywalled column filled in: NPI, practice name, specialty, full phone, city, state, ZIP, the decision-maker's first and last name, their title, the registration date and our Data Note. Nothing here is held back to make you pay for it. That is the point of the page.

One field is held back, and not for money: the street. Plenty of small practices register from home, so printing it on a public page would put somebody's home address beside their name and their phone number. It is in every paid file, and CMS will show it to you free in about ten seconds — every row below links straight to it.

Check one before you believe any of it. Every NPI below is a link straight to its official CMS page — pick three or four at random and see whether the name, the phone and the town match. It takes about ten seconds each. If a row does not match, we would genuinely want to know.

The records

20 practices across 20 states (AL, AZ, CA, CO, DE, FL, GA, IL, IN, LA, MA, MD, NJ, OH, OK, PR, TX, UT, VA, WA). One per state, at most two per specialty, so this reads as a cross-section rather than whatever the biggest category happened to be that week. Every field is printed exactly as CMS publishes it — upper case, abbreviations, occasional federal typo and all — because that is what lands in your CSV and what you will see on the registry page when you check it.

How to check one, free, in ten seconds

1. Click any NPI above. It opens that practice's official page at npiregistry.cms.hhs.gov, run by CMS, no account and no charge.
2. Compare the organization name, address, phone and the Authorized Official against the row above.
3. Do it to three or four of them. If they hold up, the other 2,600 records we publish every week were built by the same code from the same federal file — and they are days old rather than weeks.

FreshMedLeads reports public records exactly as CMS publishes them in the NPPES data dissemination file. We add no columns from any other source, and we are not affiliated with CMS or HHS. Where CMS is wrong, we are wrong in the same way — which is what the Data Note column is for.

What the Data Note means

The Data Note is the last column of every file we sell, and it is usually empty. None of the 20 rows above carries one, which is the normal case — about 89% of the rows in a paid file come through clean. We flag rows rather than quietly dropping them, because a practice with a bad phone number still has a real name, a real address and a real decision-maker — you just deserve to be told before you dial. Here is the whole vocabulary; there is nothing else it can say.

These are the CSVs the rest of the site links to. They show the exact column layout and file format you receive, but they are deliberately masked: the NPI, the phone, the street and both halves of the decision-maker's name are obscured, and any practice registered under a person's own name has its organization name withheld too. That masking is why they can sit in public. The table at the top of this page is the opposite trade — unpaywalled records, street excepted, but old.

One thing these files do not represent: the specialty cuts above exist to show the layout, not because specialty is something you buy. Every product includes every specialty — there is no filter to choose and nothing to get wrong at checkout.

The rows on this page are 42–47 days old. A paid file is not. One state or all fifty, every specialty either way, with the phone number, the street and the decision-maker's name and title filled in exactly like the table above.
